This form is to help you claim money to pay some or all of your rent and Council Tax. We will use the information you give us to work out how much Council Tax and Housing Benefit you can get.
It is important that you answer all the questions on this form and that you give us any proof of income that we ask for. If you do this we will not have to write to you for any more information and it will mean we can assess your claim correctly and quickly.
We will normally pay your benefit from the Monday after we receive this form. We will send a letter which tells you how much benefit you will get and what to do if you are not happy with the amount we decide to give you. Sometimes we can pay benefit from an earlier date if you have a good reason for not claiming earlier. If you want us to consider paying your benefit from an earlier date, please complete part 13 or 14 of this form and we will send you a backdating form.
We may use the information you put in your claim form for different purposes, for example to assess other council benefits. But we will not give it to anyone who isn't in the authority unless they are covered by the Data Protection Act or by another Act.
Please fill in as much of this form as you can but if you need further help, or you need to hand in any proof that we have asked for, please call into the benefit counter at the civic offices, or to an area office. If you cannot get out of the house, and there is nobody who can help you fill in this form, we can arrange for a member of staff to visit you.
