Request for Personal Data under the Data Protection Act (PDF 56kB)

Please enclose the following with this form:

  1. A cheque for £10.00 payable to Thurrock Council
  2. Proof of your identity. Please supply a photocopy (not originals) of one of the following:
    • The photo page of your passport
    • Your photo drivers license
    • If current employee, your Thurrock security pass

Please return the form and enclosed items to Thurrock Council's Information Management Team.
