The Homecare Reablement Service is for vulnerable people who are disabled, frail, ill or had a stay in hospital and may need support with personal care or to prepare a meal.
Thurrock Council's Homecare Reablement Service provides support to vulnerable people in Thurrock. Reablement is the name we give to short-term support designed to help keep vulnerable people safe and as independent as possible. The Reablement Team will work with service-users to learn or re-learn important tasks needed for everyday life.
A Homecare Reablement Co-ordinator will visit to discuss your needs and together with you agree goals to achieve. The Homecare Reablement Workers with then work with you to support you to achieve these goals and help you manage tasks on your own.
You will need to have had an assessment by a professional worker to discuss the support you feel you need at home. This can be arranged by contacting Thurrock Council's Community Solutions Team or, if you are being discharged from hospital, a member of the social work team will arrange this for you.
The programme will normally last for up to six weeks and will be free of charge as long as you are participating in the programme and making progress.
