If you haven't accepted your NHS COVID-19 vaccine offer
If you've been offered a NHS COVID-19 vaccine but have not accepted, you can share your reasons or concerns by completing the anonymous 2-minute online survey, below.
The survey is run by an independent research agency called Upshot.
When completing the survey, you will be given the option to help with more research by taking part in an anonymous, informal interview by phone. You don't have to take part if you don't want to, but everyone who is interviewed by phone will be paid for their time.
