Better Care Fund plan

The Better Care Fund (BCF) was created by government to improve the lives of some of the most vulnerable people, placing them at the centre of their care and support from the NHS and local authorities.

Every local area has to complete a plan, which is written jointly by Clinical Commissioning Groups and local authorities. Plans have to show clearly how they will reduce unplanned admissions to hospital.

Thurrock's Better Care Fund plan for 2017-19 shows:

  • our joint vision for the future of health and adult social care
  • initiatives to be funded

We will make sure patients, service users and their carers are involved at the earliest opportunity so we can jointly plan changes to health and social care.

We would also like your views on the future commissioning of health and adult social care, and the development of proposals for change that may affect you or your family.
