Better Care Fund plan

The NHS: Better Care Fund (BCF) was created by government to improve the lives of some of the most vulnerable people, placing them at the centre of their care and support from the NHS and local authorities.

Every local area has to complete a plan, which is written jointly by clinical commissioning groups and local authorities. Plans have to show clearly how they will reduce unplanned admissions to hospital.

Thurrock's Better Care Fund plan shows:

  • our joint vision for the future of health and adult social care
  • initiatives to be funded

We will make sure patients, service users and their carers are involved at the earliest opportunity so we can jointly plan changes to health and social care.
