Comments:

The NatSIP habilitation eligibility criteria are used as a guideline. Following referral all VIPs will be followed up and their need assessed in discussion with QTVI/families/VIP/schools/ managers. Once assessed and the need identified a programme will be developed to meet the identified needs of the VIP.

The habilitation programmes are prepared following initial assessment of need and in discussion with  the VIP/families/carers/SENCOs/managers.

Records are maintained from each session

Progress is monitored and reviewed at VI Service team meeting held fortnightly.

Habilitation outcomes are set for EHCP planning meetings and EHCP reviews.
