Dementia Support – Grant to the Alzheimer’s Society not a contract

Hospital Discharge – BTUH – 31st March 2024 also our Domiciliary Care Providers Guardian Home care and John Stanley both contracts end 31st march 2025

Social Prescribing - Not Council commissioned

Information and Advice Services -  Thurrock and Brentwood Mind Unpaid Carers Information, Support and Advice  - 31st March 2025

Advocacy - Voiceability  -  31st December 2026 with facility for a 1 + 1 year extension

Carers Support -  Noted above

End of Life Care – Not Council Commissioned

Community Champions / Wellbeing Coordinators – Local Area Co-ordinators service internal to the Council

Elderly Day Centre – internal to the Council

Befriending – Not commissioned by the Council

Footcare/toenail cutting – not Council commissioned

Non-Emergency Patient Transport – add hoc basis as required no contract

Bereavement Support – Not Council commissioned

Digital Inclusion  - Not council commissioned

Please be advised that the ICO will not consider your case until they have confirmation that you have already been through the Internal Review stage with the council.
