Transition strategy

Our 3-year strategy is for all agencies and staff who work with disabled young people between the ages of 14 and 25 in Thurrock.

It is the role of transition planning - and professionals involved in running and managing services - to be supportive in reducing the known stress and anxiety at the time of transition. The aim is to make the journey into adulthood a smooth and enjoyable one for young people, and also their parents and carers.

Agencies must work together to remove barriers and make the transition from one service to another as smooth as possible for every young person.
