Serious case reviews
Government guidance says that a serious case review should be considered when a child dies and either:
- abuse or neglect is known or suspected to be a factor in the death
- significant harm has taken place to a child or young person and there could be lessons to be learnt about the ways agencies worked together to safeguard and promote the welfare of children
Serious case reviews must report on:
- the lessons to be learnt
- how they will be acted upon
- what is expected to change
Improvements to inter-agency working must result, with better safeguarding and promotion of the welfare of children.
