Community mental health
We provide support for people with mental health difficulties.
In partnership with the Alzheimer's Society and Essex Partnership University NHS Foundation Trust (EPUT), we offer:
- advice
- information
- social activities to older people who have memory problems and their carers
- short-term intermediate care and assessments
- help so they can access appropriate social and health services
Referral to us
The following people can contact us for you:
- your family doctor (GP)
- social services
- other health professionals
- family and friends
- their agencies
We will:
- assess your mental health and social care needs
- offer you advice and guidance on how your needs may be met - this may be from within our team or by referring you to other health or voluntary agencies
- offer on-going help in the community, including support monitoring if you have a severe or long term mental health problem
